The costume was purchased at It's A Wrap! during the warehouse sale. The costume comes with an extra bloodied skirt as well as the bandanges wrapped around ROCs neck and arms (seen later in the episode. In this case, the only documentation that came with the costume is the note taped to the protective plastic bag covering the costume itself (last photo below).
